Information Services Questionnaire
Included with this issue of CONNECTIONS journal is an "Information Services Questionnaire". The aim of the questionnaire is to improve the quality and efficiency of our service to users, and we would appreciate your input.
You will find 3 types of questions on the form:
- Tick the appropriate box
- Circle the appropriate number on a scale of 1 to 5
- Give a reason or example for your choice
Due to space limitations, only a few lines have been provided for written answers. However, please feel free to add further comments on a separate sheet of paper.
